Includes a parent questionnaire for obtaining additional information about family history, onset of stuttering, changes observed over time, etc. especially for preschoolers who may be demonstrating developmental stuttering. The questionnaire can be used as an informal supplement to more formal testing measures to help with report-writing (background section) and to obtain a baseline as far as the child's fluency at the time of intake. It might also help to identify possible predictors of stuttering remission or persistence. This file also includes home suggestions that can be given to or discussed with parents. The strategies are modifications that can promote fluency (slowing rate, reducing interruptions, etc.) and also include recommendations for starting a journal to monitor/document changes to fluency (type, severity, frequency, etc.) over time.
